Terms of Use

Terms version 2026-07-22

These terms are the agreement between you and the healthcare organisation operating this service. Please read them before you create an account — creating one means you accept them.

1. Accepting these terms

By ticking the consent box when you create your account, you accept these terms and the privacy policy. If you do not accept them, please do not create an account. If you use the service as an invited caregiver, these terms apply to you too, for the parts of the record the patient has chosen to share with you.

2. What this service is

A companion for people living with a serious illness. It lets you record a consultation with your clinician's agreement, get a written transcript and a plain-language summary, keep your documents and appointments in one place, ask questions of an assistant, and — if you choose — share parts of your record with someone close to you. It supports the care you receive; it does not replace it.

3. Not medical advice, and not for emergencies

This service is an information tool. It is not a medical device, it does not diagnose, treat or monitor any condition, and nothing in it is medical advice. Summaries, answers and suggestions exist to help you understand and remember what your care team told you — decisions about your care are made with your clinician. If you have a medical emergency, or you think your condition is getting worse, contact your local emergency number or your care team immediately. Never use this app to seek urgent help.

4. Who can use it

You must be 18 or over and able to consent to the processing of your own health data. Please do not create an account on someone else's behalf: a person who wants to follow another patient's record should be invited as a caregiver by that patient.

5. Your account

Give accurate contact details — they are how we reach you and how you recover access. Choose a password you do not use anywhere else and keep it to yourself: anyone who has it can read your medical record. Tell the data controller straight away if you think someone else has access to your account. You are responsible for what is done through it.

6. Recording consultations

Before you start a recording, make sure your clinician has agreed to it. Recording a conversation without the agreement of the people taking part may be unlawful where you live, and that responsibility is yours. Only record consultations you take part in, and do not pass a recording, transcript or summary to anyone who was not entitled to be there.

7. Fair use

Use the service for your own care. Do not try to reach other people's data, break or circumvent its security, overload it, copy it wholesale, or upload content that is unlawful or that you have no right to share. Do not attempt to make the assistant give medical advice or bypass its safety rules.

8. Automated features and their limits

Transcripts, summaries, document readings, suggested care-path steps and assistant answers are produced by artificial intelligence. They can be incomplete or wrong, and they can miss things that were said. Treat them as notes to be checked, never as a record of fact — verify anything that matters with your care team. Steps suggested for your care path remain proposals until a clinician confirms them.

9. Sharing with a loved one

Sharing your record with a caregiver is optional and entirely your choice. You decide who you invite, and you can withdraw that access at any time in Settings — which immediately cuts off their access and cancels any pending invitation. Bear in mind that what a caregiver has already seen cannot be un-seen.

10. Availability

We work to keep the service running, but we cannot promise it will be available without interruption. Maintenance, outages and connection problems happen. Some features keep working offline and sync once you reconnect, but please do not rely on the service being reachable at a moment that matters medically.

11. Content and ownership

The application, its design and its software remain the property of their owner. Your medical content stays yours: it is processed only to provide the service to you, as set out in the privacy policy, and you can export it at any time.

12. Responsibility

The service is provided as it is. To the extent the law allows, the operator is not liable for decisions taken on the basis of an automated summary or assistant answer that you did not check with your care team, nor for loss caused by content you chose to share with someone else. Nothing in these terms limits liability that cannot be limited by law, including for personal injury caused by negligence.

13. Ending your use

You can delete your account at any time in Settings. This erases your medical record with it and cannot be undone — export your data first if you want to keep a copy. The operator may suspend or close an account that breaches these terms or is used in a way that endangers other users or the service.

14. Changes to these terms

These terms carry the version number shown at the top of this page, shared with the privacy policy. When a change materially affects you, we ask you to review and accept the new version the next time you sign in. Continuing to use the service after that means you accept it.

15. Applicable law

These terms are governed by the law of the country in which the data controller is established, and any dispute is heard by the courts of that country. If you are a consumer, this does not deprive you of the protection of the mandatory rules of the country where you live.
